For example this sentence 
I have a grandmother, her name is Vilma, I also have a grandfather, and his name is Claudio. My brother is his favorite Grandson and my sister is *his or her favorite Granddaughter
 In this case What is the correct? *his or her

It depends on who is the object. If your object is grandfather's favorite Granddaughter, it will be "his", otherwise it is "her"

You must use THEIR, because you are talking about both, your grandpa & grandma,  they both have a preferred grandson and granddaughter..    

I have a...      .  My brother is their favorite grandson and my sister is their favorite granddaughter.

should be "their" because you are talking about a grandson of your grandparent
